Medical Records Release Policy

We are unable to send any medical records out of our office without a signed authorization by the patient or their guardian.  Once a completed form is received, the request will be promptly processed.  The only exception to this is when we refer you to a specialist for care and if a school or daycare sends us a faxed request for immunization records.
