Upon your arrival you will be asked to furnish us with your health insurance information. This will allow us to have our records up to date. Each patient is required to present their health insurance card at each visit. Failure to present your card will result in a proof of insurance form to be signed and payment will be expected. Each patient or their parent or guardian is ultimately responsible for any and all charges incurred. With this in mind, we kindly ask that you keep our office up to date on each and every health insurance coverage that you have. Any patients that do not have health insurance are required to pay for their service at the time of service. We offer a discount for all cash services paid in full. We presently accept many insurance plans including Kansas Medicaid.
